When the need arises for home health care services and hospice care, count on Aurora Health Care for solutions.

Aurora Visiting Nurse Association of Wisconsin offers a wide range of home care services for patients of all ages who need nursing, therapy or other support services in their homes. Home care promotes independence while keeping patients connected to a complete continuum of care backed by the full resources of Aurora.

For those whose life expectancy is limited, hospice offers a special way of caring through a wide range of services to benefit the terminally ill patient and his or her family, including children. Our hospice team of physicians, nurses, social workers, clergy, aides and highly trained volunteers work together with the patient and family to develop a treatment plan focused on keeping patients pain-free and comfortable.

More than 95 percent of our patients are able to be cared for in their own home through hospice care. When in-home hospice care is not an option, the Aurora VNA hospice team can follow a patient into any care environment—a nursing home, assisted living center or hospital. In the Milwaukee area, we offer hospice care at the Aurora VNA Zilber Family Hospice residence, where you’ll experience compassionate care delivered in a secure and homelike setting.
